My child’s whole story, birth to now

Child’s name ______________________   Born ____________   Filled in by ______________________   Date ________

How to use this: walk your child’s life in order. Pin everything to an age, a season, or a grade. When two things happened in the same month, write which came first. Nothing is too small — the odd little things are often where the answer is. Pull out old texts, calendars, and photos to find dates.

Every home, in order

Ages lived there · city or country · leaks, water damage, basement rooms, musty smell, renovation

Pets — scratches or bites? Ticks — summer homes, camps, grandparents’, hikes

Better away from a place, worse coming back?

Pregnancy and birth

Illnesses, antibiotics, medicines, vaccines in pregnancy · where you lived, any renovation

C-section, antibiotics in labor, NICU, breathing or feeding problems

Baby and toddler years

Ear infections and how many antibiotic courses · eczema · reflux

Milestones (crawling, walking, first words) · sleep · temperament

The small odd things · any reaction to a medicine or vaccine — what happened, at what age

Each year after

One line per year: illnesses, antibiotics, school, home, family, how your child was doing

Was there a time your child was suddenly different?

The change

Date or season · overnight, over days, or slowly? · the very first things you saw

The days, weeks, and months before: illness, fever, strep at school, injury, a move, a new medicine, a stressor, mold

Your child’s own words (“Mom, I can’t stop”)

Every flare since

Date · what came right before (a cold, a loose tooth, a visit somewhere) · what you tried · what happened

Who else in the house was sick?

The best stretches

When was your child most like themselves? What were they on, or doing, then?

Right now

The three hardest things this month · what sets things off · how often they poop

Everything you’ve tried

Antibiotics, steroids, IVIG, psychiatric medicines, supplements, herbs, homeopathy, diets, therapies. Include the brand and dose. Write what happened in the days after — better, worse, or strange (a child who suddenly sleeps through the night on an antibiotic is telling you something).

When (age / date)What (name, brand, dose)How longWhat happened — better, worse, strange

Every test that’s been done

Blood work, swabs, stool, urine, scans, sleep studies. “Normal” results matter too — keep the reports.

WhenTestResultDo you have the report?

Questions worth asking yourself

  • Did anyone swab for strep at each sore throat or fever? Has anyone else in the house been tested?
  • Has anything ever made my child worse within hours or days of starting it? What did I stop, and what happened?
  • What comes right before the rage, tics, or accidents come back?
  • Is there anyone my child suddenly can’t stand to be near?
  • Do I keep any kind of log — notes app, texts, calendar, a homeopath’s notes? Bring it.
