Plan Bfor PANS
Real Plan B cases, shared with each family’s written consent. Names and identifying details changed — “Theo” and “Max” are pseudonyms. Geography, dates, and exact lab values have been removed or generalized so no case can be matched back to a child. The clinical story is preserved.

Real case examples

The integration no single specialist had done.

These are real children. Their families gave written consent to share a fully de-identified version of their case — because what happened for them happens for almost every complex PANS kid: a thick folder of testing, seen piece by piece by different specialists, that nobody had ever read together.

The data to solve these cases largely already existed. The missing step was integration — reading every lab against its own range, across every framework at once, and asking what they say together. That’s the work no human specialist has the time to do across infection, immunity, methylation, the gut, and the nervous system simultaneously. It’s the whole point of a Plan B synthesis.

Case example · “Theo”

Three drivers, stacked.

A teenager with years of unclearable infection that no rotation of antibiotics could touch. Reading his whole file together surfaced three drivers stacked on top of each other — and the order to address them in.

  • An active tick-borne infection picture
  • A CVID-spectrum immune deficiency — recognized, treatable, and covered
  • A methylation / detox block that made aggressive protocols backfire

Why nothing before had stuck: you can’t just kill the infections. Immune support first, fix the cleanup engine second, kill third.

Case example · “Max”

Not one disease — a convergence.

One of the most layered pictures we’ve worked through. A long history, many diagnoses, years of treatments that mostly hadn’t landed — until every lab was read top-to-bottom and the pieces were finally integrated.

  • An unaddressed post-strep autoimmune signal, years in the making
  • An undermethylator phenotype that made standard supplements backfire
  • A heavy allergy / histamine load + a chronic viral signature

The reframe for his family: he is not unfixable. He is genuinely complicated — and, finally, legible.

Each case is one child. Nothing in them is a protocol for anyone else, and everything is framed as questions to investigate with a qualified practitioner. Plan B is a navigation and integration service, not a medical provider.
